Enroll in Benefits

You are eligible for company benefits if you are a regular full- or part-time employee who works at least 20 hours per week.

Your Eligible Dependents

EAB allows you to enroll your eligible dependents, who include:

  • Your legal spouse (from whom you are not legally separated or divorced)
  • Your domestic partner*
  • Your dependent children up to age 26 (including legally adopted children, stepchildren, requirements, and any child placed in the home for legal adoption or guardianship)
  • Your dependent children over age 26 who are physically or mentally handicapped and unable to care for themselves.

*Your Domestic Partner must meet the guidelines set in the affidavit. Your domestic partner and you will need to complete the affidavit and submit it to benefits@eab.com for review within the 31 day period of the qualified life event, new hire enrollment, or open enrollment.

If you need to make a change to your existing coverage, or terminate coverage for a domestic partner and/or children of domestic partner, please complete this notice of change. The completed form should be submitted to benefits@eab.com within 31 days of the change taking place. Please note that notarization of this document is required prior to submitting it to Benefits.

New to EAB?

Welcome to the team! As a newly hired employee of EAB, you have 31 days from your date of hire to elect your Health & Welfare Benefits. Remember: You MUST either elect or decline benefits in Dayforce within 31 days of your date of hire.​ You are strongly encouraged to review your choices carefully. You will not be permitted to make changes during the year unless you experience a qualified event change.

Making Changes to Your Benefits

EAB offers a Section 125 Cafeteria Plan that allows you to deduct your contributions for medical, dental, and vision insurance on a pre-tax basis. This means EAB reduces your pay by the amount of employee contributions before the taxes for Federal, State, and Social Security are calculated. The pre-tax deduction lowers your taxable income and the amount of taxes you pay.

The tax laws associated with Cafeteria Plans mandate that unless you experience one of the Qualifying Events described below, you will not be able to make changes to your elections until the next Open Enrollment, during the fall of each year.

Documentation is required when making a benefits changes due to qualifying life event. Documentation should note the person(s) covered, and the date coverage was either lost, or verify that coverage is active.

If you would like to add your domestic partner and you are not legally married, you are required to complete the affidavit. The completed form should be submitted to benefits@eab.com within the 31 day period of the qualified life event, new hire enrollment, and open enrollment.

Qualifying Event Changes

  • Marriage or divorce
  • Birth, adoption or placement for adoption of a child
  • Death of a spouse, domestic partner or child(ren)
  • Spouse commences or terminates employment
  • You, your spouse’s employment status changes from full-time to part-time or vice versa
  • Court decree requiring coverage for your dependent children
  • Entitlement to Medicare or Medicaid
  • Dependent child ceasing to be an eligible dependent such as attaining maximum dependent age
  • You turn 26 and age off your parents plan
  • You, your spouse takes an unpaid leave of absence
  • Your spouse has a significant change in health coverage directly attributable to your spouse’s employment
  • Entitlement or loss of coverage under premium subsidy plans from a state

Important Deadline!

Remember: If you experience a Qualifying Life Event, you must take action in Dayforce within 31 days of the event.

Initiate Changes in Dayforce

  1. Update Insurance 
    • Initiate a Benefits Change in Dayforce for the event date.
      • Review this guide or the instructions below on submitting or initiating a benefit change in Dayforce.
      • Initiate the change by going to the Dayforce home page, click on Benefits>Change Benefits> Select the appropriate Qualifying Life Event based on your situation.
      • The Event Date will be the date of when the Qualifying Life Event occurred.
      • Attach proof documentation. Your changes will not be approved until The Benefits Team receives proper documentation.
  2. Update Beneficiary Information
For Marriage and Divorce benefit events, you will want to update the following:
  1. Legal Name Change (if applicable)
    • You must update your name with the Social Security Administration.
    • Initiate a Legal Name Change in Dayforce.
    • Follow the attached guide to complete your legal name change in Dayforce.
    • Submit documentation of your name change in the event.
      • Acceptable Documents – driver’s license, driver’s license receipt, or SSN Card.
      • If you wish to submit your SSN card, please let us know so we can send you a secure Box folder. DO NOT send your SSN card or email your SSN card.
    • Update your Email Address – your email address will not be automatically updated. If you want your email changed, please send an email to IT at techsupport@eab.com.
  2. Update Tax Status 
    • Federal– Update your Federal Tax withholding in Dayforce.
      • From your Dayforce profile, click on Forms > Tax Forms.
    • State– Update your tax withholding in Dayforce.
    • If applicable, complete a paper DC Non Resident form

When does coverage Start or End?

  • Coverage for any additions to EAB’s plans will begin on the event date.
  • For any benefits that are waived, coverage will end on the day prior to the event date.
