Voluntary Life

We are happy to provide you with the opportunity to purchase additional life and/or AD&D insurance to meet the financial needs for you and your family in the event of an untimely death.

You can purchase additional life insurance for yourself in $10,000 units up to a maximum of 5x your annual compensation or $1,000,000, whichever is less. If you apply for additional life insurance within 31 days of your hire, you are automatically guaranteed coverage up to $200,000. If you apply for additional life insurance after 31 days from your hire, all amounts will require evidence of insurability.

Spouse coverage is available if you purchase additional life insurance for yourself. You may elect coverage for your spouse in increments of $5,000 units up to $500,000 not to exceed 100% of the employee’s coverage amount. If you apply for Spouse life insurance within 31 days of your hire or 31 days of your marriage, they are automatically guaranteed coverage up to $25,000. Any amount above $25,000 will require evidence of insurability. If you apply for spouse life insurance after 31 days from your hire or more than 31 days from your marriage, all amounts will require evidence of insurability.

Coverage is available for your child or children if you purchase additional coverage for yourself. You may elect increments of $2,000 up to a maximum of $10,000. All amounts are guaranteed.  Only one premium is required for single or multiple child coverage.
