Life and Accidental Death & Dismemberment (AD&D) Insurance
EAB provides a group Life/AD&D insurance benefit to all benefits eligible employees at no additional cost to you.
- Life Benefit: EAB provides you a basic life benefit of $50,000.
- AD&D Benefit: In the event of an accidental death, you will receive a benefit of $50,000.
- Coverage End Date – Your coverage will be cancelled as of your last working day.
- Coverage Portability – You may be eligible to convert or port your basic and your voluntary life coverage(s) to an individual whole life insurance policy.
- Portability Deadline – You must complete an application by contacting Prudential and apply for these options within 31 days of your coverage termination.
