Parental Leave Program
EAB recognizes that state and federal parental leave laws only provide for unpaid leave, and only after employees meet certain eligibility criteria. To make it easier for all employees to use leave to recover from child birth or bond with their new child, EAB is providing a paid parental benefit (“PPB”) to employees following the arrival of a new child through birth, adoption or surrogacy. You can review the paid parental benefit and all leave options in our Employee Handbook, starting on page 20.
PPB runs concurrently with federal FMLA leave, as applicable, as well as any State or Local leave laws that provide FMLA-type leave. PPB is coordinated with any applicable statutory or voluntary sources of monetary benefits so that qualifying employees may not receive more than one hundred percent (100%) of their base compensation at any time.

If you are pregnant, adopting or going through the surrogacy process, please reach out to EAB Global’s leave vendor, Prudential as soon as you are aware of the need for a leave of absence. Prudential will be able to answer all of your questions about EAB Global’s policies and federal and state leave laws. Your Prudential dedicated leave administrator will determine your eligibility for leave and communicate with you and EAB Global throughout your leave until your return to work. Preparing for Leave
- Reach out to Prudential to initiate a leave of absence request as soon as you become aware of your need to take leave. Prudential’s contact information is below:

- Prudential will provide you with an Introductory Letter. Read this letter carefully as it will provide detailed information about your rights and obligations during leave as well as income replacement benefits that may be available to you.
- Important Deadline: Complete the Healthcare Provider Certification Form and return the form to Prudential no later than 30 days prior to your expected leave start date.
- Important Deadline: Sign, date, and return the Pay Request and Insurance Continuation Acknowledgement Form to benefits@eab.com no later than 30 days prior to your expected leave start date.
- Please log any time off you wish to use during your leave in Dayforce– reference this guide for steps on logging PTO.
- Decide if you will be using EAB Global’s Phase Back to Work program and discuss it with your manager.
- Set up your automatic replies in Outlook while you are on leave
While on Leave
- Important Deadline: Notify Prudential the date your baby arrived and your method of delivery within 5 days.
- Important Deadline: Change Your Benefits– You must take the necessary steps to make changes to your benefits, within 31 days of the birth of your child.
- Initiate the change by going to the Dayforce home page, and follow the instructions in this guide.
- The Event Date will be the date of birth of the child/adoption of child.
- Attach proof documentation. Your changes will not be approved until The Benefits Team receives proper documentation.
- Please reference this guide for additional help.
- Important Deadline: File for any State benefits – Prudential will provide you with information on available benefits and how to apply if you qualify.
- You should file your claim as soon as possible but no later than 30 days after your qualifying event occurs.
- States with Paid Family Leave Programs: California, Colorado, Connecticut, Delaware, Maine, Maryland, Massachusetts, Minnesota, New Jersey, New York, Oregon, Rhode Island, Washington, and Washington D.C.
- Important Deadline: File your claim for Short Term Disability (STD) with Prudential by calling 833 458-4782, EAB Global’s control number is #70313.
- We strongly recommend completing this step as soon as possible following the start of your leave to avoid interruptions to your pay.
Returning from Leave
- Confirm your return-to-work date with Prudential within 2 weeks of your scheduled return date or when you are medically released to return to work.
- If plans for your return to work have changed, please notify Prudential and the Benefits Team at Benefits@eab.com as soon as possible.
- Important Deadline: Return your completed Fitness for Duty Certificate back to Benefits@eab.com on (or prior to) the date you return to work.
- You should complete the Employee Information section; your doctor must complete the Health Care Provider Information section.
- Remember to block time on your calendar to pump, if necessary. If you are in office, all EAB offices have designation Lactation Rooms for nursing mothers. To request access to these rooms, please book time through Outlook, further instructions are in the Resources for Parents section below.
Phase Back to Work Program
Our phase back to work program allows for an easier transition back to work.
- 2 Week Phase Period –Employees who return from parental leave of at least 4 consecutive weeks are eligible for a 2 week phase back period starting on the normally scheduled return to work date.
- Week 1 –For the first week (5 business days), employees may work 50% of their normal work schedule.
- Week 2 –For the second week (5 business days), employees may work 75% of their normal work schedule.
- Example 1:
- Week 1: Employee works a half day (4 hours) each day; Monday – Friday 8:30am-12:30pm
- Week 2: Employee works 6-hour days each day; Monday – Friday 8:30am – 2:30pm
- Example 2:
- Week 1: Employee works two full days and one half day; Monday – Tuesday 8:30am -5:30pm, Wednesday 8:30am – 12:30pm, off Thursday – Friday
- Week 2: Employee works three full days and one 6-hour day; Monday – Wednesday 8:30am – 5:30pm, Thursday 8:30am – 2:30pm, off Friday
During the phase back period employees will receive 100% of their regular base salary and will return to full active status on their return to work date.
- Employees who return from a qualifying Family Medical Parental Leave must work with their manager prior to their return to determine a mutually agreeable schedule for the 2 week phase back period, if they qualify.

EAB partners with SupportLinc EAP to bring you resources on family topics such as adoption, child-care, children’s health, pregnancy, parenting and much more. The EAP is available to all EAB employees and their immediate family members, whether or not your health insurance is through EAB.

Aetna Breast-Feeding Pump
If you are on the Aetna medical plan through EAB, you are eligible for a manual or standard electric breast pump at no charge with each pregnancy. Visit the Aetna website for more details.
Aetna Lactation Assistance – SimpliFed
Through EAB’s partnership with Aetna, members in the Medical plan will have access to a virtual breastfeeding and baby feeding support service provided by SimpliFed. Both the CDHP and PPO plans cover up to 6 appointments with no prior authorization requirements.

With this service, you have access to 1:1 virtual support from certified baby feeding experts that can help with:
- Breastfeeding
- Latch and positioning
- Low supply or oversupply
- Cracked or sore nipples
- Pumping
- Formula feeding
- Formula and bottle prep; Pace feeding; Combo feeding

Aetna Lactation Assistance – In Person
Through EAB’s partnership with Aetna, members in the medical plan will also have access to in-person lactation consultations. Both the CDHP and PPO plans cover up to 6 appointments per 12 months with no prior authorization requirements and no deductibles or copays. Any additional visits will incur cost share for a specialist visit.

Family Forming Benefits & Adoption Assistance
Family Forming Benefits
If you are enrolled in the Aetna medical plan, you are eligible for fertility coverage through Progyny Fertility Benefits. Progyny is a best-in-class provider of fertility and family-building benefits, with a modern approach to reproductive health coverage. With Progyny, there are no heteronormative requirements or barriers to accessing care. Additionally, precertification is not required, and doctors have the flexibility to provide the most effective treatment the first time.

Adoption Assistance
In order to support eligible employees who are building their families through adoption, the Firm offers an Adoption Assistance Reimbursement Program. Adoption is defined as steps taken through legal procedure to assume parental responsibility for an unmarried child younger than eighteen (18) years of age and who is not the child of the employee’s spouse or domestic partner.
Reimbursement assistance is available for up to $5,000 per adoption event with a lifetime maximum limit of two adoption events per employee’s tenure with the company.
Eligible Employees include regular full- or part-time employee who works at least 20 hours or more per week (temporary employees and interns are not eligible for this benefit).
Eligible Expenses include:
- Adoption agency placement fees
- Attorney fees for the adoption
- Court cost for the adoption
- Reasonable & necessary transportation and lodging associated with the adoption
