Prudential Accidental Injury, Critical Illness and Hospital Care Insurance

Prudential’s Accidental Injury, Critical Illness and Hospital Care insurance can supplement your Prudential medical plan and provide you and your family with the coverage and additional financial protection you may need for expenses associated with an unplanned covered accident, illness or hospitalization.

These plans pay benefits directly to you. What you do with the money is all up to you. 

Our plans allow domestic partners on eligible Medical, Dental, Vision, Life Insurance and Supplemental health benefits. Review thee Qualifying Life Events page to download the required affidavit that should be submitted to benefits@eab.com.

Not planning to enroll in an Prudential health plan this year? Not a problem. You can still enroll in Prudential Accidental Injury, Critical Illness and/or Hospital Care if you’re eligible for these benefits. 

 

Voluntary Critical Illness Plan

The Prudential Critical Illness Plan pays a lump sum cash benefit directly to the insured upon the first diagnosis, after the coverage effective date, of a covered condition from a single list of eligible conditions. A sample of covered conditions include Cancer, Heart Attack, Stroke, Multiple sclerosis, Lupus, Type 1 Diabetes, Parkinson’s Disease, Renal (kidney) failure, Major organ failure, Paralysis, Amyotrophic Lateral Sclerosis (Lou Gehrig’s disease), Blindness and Coronary Artery Disease (surgery).   

There is also a Health Screening Benefit of $100 per covered person for eligible preventive care screenings.  Examples include, but are not limited to, eye exam, mammography, bone marrow testing, pap smear (for women over age 18), PSA test, skin cancer screening, colonoscopy, and certain blood tests.

Voluntary Accidental Injury Plan

The Prudential Voluntary Accident Plan pays a lump sum cash benefit directly to the insured  for a broad range of accident treatments and conditions, based on the schedule provided below. 

This is a group accident 24-hour insurance policy. Benefits provided are not intended to cover all medical expenses. This is not a substitute for comprehensive health insurance.

There is also a Health Screening Benefit of $100 per covered person for eligible preventive care screenings.

Hospital Care Indemnity

The Prudential Hospital Indemnity plan pays a lump sum directly to the insured for a planned or unplanned hospital stay for an illness, surgery, injury or having a baby. There is a lump sum benefit for admission and a daily benefit for a covered stay.

  • Hospital Admission: benefits are payable for the initial day of your stay. There are no maximum stays per plan year, but they must be separated by 30 days in a row. Covered person must be admitted as an inpatient to the hospital. Excludes treatment in an emergency room or provided on an outpatient basis, or for re-admission for the same covered injury or illness (including chronic conditions). 
  • Hospital Intensive Care Unit (ICU) Stay and Hospital Stay (Daily): benefits are payable per day, limited to 30 days per plan year. Covered person must be admitted as an inpatient and confined to the hospital. If eligible for both benefits, only one (1) benefit will be paid per day, whichever is greater. 
  • Other payments: There may be payments for Observation Units, Newborn Routine Cars and other daily payments for substance abuse, mental disorders and rehabilitation unit stays. 

Your coverage includes a health screening benefit of $50 for Hospital Indemnity Insurance that is payable once per calendar.

Need to File a Claim?

You can file a claim on the mobile app or member portal. Click on the flyer here to learn more.

  • Coverage End Date – Your coverage will be cancelled at the end of the month you terminate. 
  • Coverage Portability – You may be eligible to port your Prudential Accident, Critical Illness and Hospital Care plans into individual policies. 
  • Portability Deadline – You must complete the Portability Coverage Election Form and return this to Prudential (along with the payment for the first premium for the portability coverage) no later than 30 calendar days after your coverage under the policy ends.
